Accidents, Incidents & Near Misses

To supplement KSU Worker's Compensation Program and the KSU Employer's Report of Accident document reporting process, the College of Agriculture requests that all departments and staff implement the Report of Accidents, Incidents, and Near Misses Standard of Practice.

For accidents, incidents, and near misses occurring in the College of Agriculture, please fill out our Investigation Form in addition to KSU's Employer's Report of Injury form and submit it to the EH&S Office.

Root cause analysis is a structured approach that can assist the investigators in identifying underlying factors or causes of an incident.  Understanding the contributing factors or causes to system or human failure can help to develop actions that result in corrective measures.  The 5 Whys is a simple problem-solving technique to help get a the root cause of a problem quickly.
